In accordance with our Behavioral Based Safety (BBS) procedures we were able to recognize that most near-miss incidents in disregard of their Maximum Reasonable Outcome (M.R.O) were as a result of people doing jobs they were not trained to perform. To put controls on such risks we have become stricter in our recruiting process of all new employees.
